Making a Donation

State law requires us to use our best efforts to collect and report the name, mailing address, occupation, and employer of individuals whose contributions (if retired, former occupation and employer) exceed $50.

Please submit electronic donations by using the Donations form located to the right of this page.  When completing the form, please ensure that you include all the information requested.

Donations by Mail

Alternatively, you may mail your contribution directly by printing and completing the following form and submitting a personal check (i.e. no business checks) to the following address.
